Trigger toe is a movement-related condition involving the flexor hallucis longus (FHL) tendon that bends the big toe. Normally, this tendon glides smoothly, but when it becomes irritated or inflamed, movement can become uneven. This may cause the toe to catch, click, or temporarily lock during motion.
This condition is commonly seen in ballet dancers, as repeated pointing (en pointe or demi-pointe) places significant strain on the tendon, increasing the risk of irritation and restricted movement.
Over time, ongoing tendon tightness or imbalance can also contribute to structural changes in the toe over time. Conditions such as hammertoes develop when opposing muscles and tendons no longer work in balance, leading to abnormal bending at the joints.

- High forefoot loading (e.g., ballet, jumping sports)
- Tight or restrictive footwear

Trigger Toe & Tenotomy Treatment Options
Treatment is based on the severity of your symptoms and how much the tendon movement is restricted. In earlier stages, treatment focuses on reducing irritation and improving tendon movement. This includes footwear changes to reduce pressure, padding or offloading techniques, custom orthotics to improve mechanics, activity modification, and stretching and strengthening. These approaches aim to restore smoother tendon gliding and prevent worsening symptoms.
When a Procedure May Be Recommended
If symptoms persist or begin to interfere with daily activity, a minor release procedure known as a tenotomy may be considered. If the big toe catches, locks, or moves painfully due to tendon or soft tissue problems, trigger toe correction may help restore smoother motion and reduce discomfort.
A tenotomy is a minimally invasive procedure that involves releasing a tight or restricted tendon to improve toe alignment and function. The toe is then able to move more freely and the catching or locking sensation is reduced.

Supportive footwear is recommended during recovery and follow-up care ensures the toe is healing well and moving properly.
