Big Toe Arthritis Care
Don’t let stiff big toe joints limit your steps.
Pain When You Bend Your Big Toe?
If walking, running, or simply bending your big toe causes pain, you might be dealing with hallux rigidus — a form of arthritis in the joint at the base of the big toe. It’s a common but often underdiagnosed condition, especially in adults over 30, and it tends to get worse without treatment.
At our podiatry clinics in Toronto, we take a non-surgical approach to relieve your pain and restore function. From custom orthotics and joint mobilization to shockwave therapy and footwear adjustments, we’ll help you walk more comfortably — and refer you to orthopedic care if surgery is the next step.
What Is Hallux Rigidus?
Hallux rigidus is a degenerative condition affecting the metatarsophalangeal (MTP) joint at the base of the big toe. It’s essentially a localized form of osteoarthritis, where the cartilage in the joint wears down over time. This causes pain, stiffness, swelling, and eventually, a limited range of motion — often making it painful to walk, run, or wear regular shoes.
Common causes include:
- Joint overuse or trauma (e.g., stubbing your toe or sports injury)
- Foot structure abnormalities (such as flat feet or high arches)
- Genetics or family history
- Repetitive strain in certain occupations or sports
Left untreated, hallux rigidus can progress to bone spur formation and complete joint immobility.
Hallux Rigidus Symptoms
You may be dealing with hallux rigidus if you notice:
- Pain or stiffness in the big toe when walking, standing, or bending
- Swelling around the joint
- Difficulty wearing heels or shoes with tight toe boxes
- A “hard bump” on top of the toe joint (bone spur)
- Reduced range of motion in the toe
- Pain in the arch, ankle, or knees from compensating
Symptoms often worsen over time and may limit your ability to stay active if untreated.
Hallux Rigidus Treatments
At Step Right Foot Clinic, we specialize in conservative treatments designed to reduce pain, slow progression, and help you maintain mobility. Our approach may include:
- Custom Orthotics: Designed to offload pressure from the big toe joint and restore proper foot mechanics.
- Shockwave Therapy: Extracorporeal Shockwave Therapy (ESWT) is ideal for chronic pain by using sound waves to stimulate healing and reduce inflammation in stubborn arthritic joints.
- Joint Mobilization: Gentle, targeted manipulation techniques help preserve range of motion and reduce stiffness in early to moderate cases.
- Footwear Modifications: We guide you toward rocker-bottom soles, stiff-soled shoes, and wide toe boxes to reduce joint stress during walking.
- Activity & Movement Guidance: We’ll coach you on low-impact movement strategies to stay active without flaring up your condition.
- Surgical Referral (Advanced Cases): In Ontario, orthopedic surgeons perform surgical correction such as joint debridement, fusion, or joint replacement. We’ll refer you if non-surgical options are no longer effective.
